The Tattoo Forms You Need
First, let’s start with a list of the forms you’ll need to provide your customers. They will ensure that you’ve dotted all your i’s and crossed all your t’s.
Tattoo Consent Form
This is one of the most important forms a customer can sign. It might seem obvious that they want a tattoo at the time, but you never know if they’ll turn around and say they didn’t actually want it.
To cover your business, you need to have every customer sign a tattoo consent form. This will tell them everything they need to know about health and safety concerns, regarding both the tattoo itself and your tattoo parlor. By signing this form, they indicate that they’re fully informed of these risks and that they accept them.
You’ll also need tattoo consent forms for minors in some cases. This is pretty similar to a regular consent form, except the minor will need their parent or legal guardian to sign off on the tattoo as well.

Tattoo Release and Waiver Form
Customers will also have to sign a tattoo release form, or tattoo waiver form. They’ll answer a series of questions with either “yes” or “no”. The purpose of this form is to make sure that your business isn’t liable for any damages that might occur after the clients have left the premises.
For example, if your tattoo artists give them clear instructions on aftercare, but a client ignores them, they can’t turn around and sue you if their tattoo becomes infected.
On this release and waiver form, customers will confirm any health conditions they might have, such as pregnancy, hepatitis, allergies, etc. They’ll also answer whether they’ve consumed alcohol, food, or anticoagulants in the last 24 hours. This is to ensure that they’re in good health and that they’re physically ok to proceed with a tattoo.
All of these questions will remain confidential. Answering “yes” to any of the questions won’t necessarily prevent the clients from getting a tattoo, but the information will allow you to adjust your services so they get a tattoo as safely as possible.
Customers will also confirm they’re over the legal age to get a tattoo. If not, then they’ll have their parent or guardian confirm their age on the form.
Body Piercing Form
Many tattoo parlors also have piercers on-site. If this sounds like your business, then you’ll need body piercing forms as well.
This form is very similar to the tattoo ones. Basically, clients will confirm that they want to get a piercing and are aware of the risks involved. They’ll also answer some questions on their medical history so your piercers are confident that their clients are in good shape to go ahead with a piercing.
